TEAM 360 ELITE — TRYOUT WAIVER & RELEASE

I, the undersigned parent or legal guardian of the player named in this registration, give permission for my child to participate in Team 360 Elite basketball tryouts and related activities.

I understand that basketball is a physical sport involving running, jumping, and contact, and carries a risk of injury, including serious injury. I voluntarily assume all risks of participation.

I release and hold harmless Team 360 Elite, its coaches, staff, and volunteers, and the owners/operators of any facility used for tryouts, from any claims arising out of my child's participation, except in cases of gross negligence.

I confirm my child is physically able to participate and I have disclosed relevant medical conditions above. In an emergency, I authorize basic first aid and, if I cannot be reached, emergency medical treatment for my child.

Tryouts are free — no fee to participate in tryouts. Season costs, if your child is offered a roster spot, will be shared transparently before anyone commits.
